The worksheet is to be provided by you to your client. Please make sure that your client fills out every part of the worksheet pertaining to the type of work they are having done. If they are having a new design constructed, please make sure that they fill out the site examples area with a minimum of 4 URL's. Keep in mind that this area is for design examples. They do not need to give examples of their competitors sites. We are just looking for designs that they like the most out of any site they have visited.
